Terms of Service

These Terms of Service ("Terms") govern access to and use of MedFormkit. By creating an account or using the platform, you agree to be bound by these Terms on behalf of yourself and, where applicable, your clinic or facility.

MedFormkit lets clinical teams build assessment forms, assign them to patient files, capture submissions, and generate charts and trend views from that data. Some features surface preliminary AI pattern flags to help clinicians prioritize review.

MedFormkit is a data and workflow tool. It is not a medical device and does not provide medical diagnoses, treatment recommendations, or clinical decisions. All clinical judgment remains with your licensed staff.

You must be authorized to act on behalf of your clinic or facility to create an account. You are responsible for maintaining the confidentiality of login credentials and for all activity under your account.

You agree not to misuse the platform. This includes not attempting to access data outside your clinic's account, not reverse engineering the service, and not using MedFormkit to store data you are not legally authorized to collect.

Your clinic is solely responsible for the accuracy of data entered into forms, for obtaining any necessary patient consent, and for how clinical decisions are made based on information in the platform.

MedFormkit stores and organizes the data your team provides. We do not independently verify clinical accuracy of submissions.

Paid plans are billed on the cycle selected at signup. Fees are non-refundable except where required by law. We will provide reasonable notice before any pricing changes take effect for existing subscriptions.

MedFormkit and its underlying software, design, and branding are owned by us. Your clinic retains ownership of the data it enters into the platform.

AI-generated pattern flags are preliminary and intended only to draw attention to data that may warrant clinical review. They are not a diagnosis and must not be treated as a substitute for professional clinical judgment.

The platform is provided on an "as is" and "as available" basis. To the maximum extent permitted by law, MedFormkit is not liable for indirect, incidental, or consequential damages arising from use of the service, including decisions made based on data or flags generated by the platform.

Either party may terminate an account as described in the applicable subscription agreement. Upon termination, we will provide a reasonable window to export clinic data before deletion.

These Terms are governed by the laws of the jurisdiction in which your clinic's account was established, unless otherwise agreed in a separate written contract.

We may update these Terms from time to time. Continued use of the platform after changes take effect constitutes acceptance of the revised Terms.
